HCP services are provided in what is called a “Consumer Directed Care Model” which gives you greater control over your life by allowing you to make choices about the types of care and services you receive using the funds provided for your package.
To receive a Home Care Package, you will need to be assessed by the Aged Care Assessment Team, or ACAT. These teams are based at major hospitals and your GP can arrange for your assessment. Provided that you are eligible, ACAT will then refer you to a local HCP provider, such as Astley Care.
Astley Care’s Package Coordinator will arrange a home visit and work with you to develop a support plan based on your needs and goals. It will be designed to provide you with support that focuses on the things you can’t do yourself and may include services like assistance with bathing, dressing, basic housework, meal preparation, transport, socialisation and respite.
While Home Care Packages are very comprehensive, if you require additional services not covered by your package, you are able to “top it up” using your own funds.
